How many children do you have? Gender Nationality Second Nationality (optional) Full Address: street, city, region, postcode Country of Residence Are you applying for an Erasmus Mundus Studentship? If yes, which category are you applying to? (category A or category B) ----------------------------------------- Stage 2 Qualifications You will be required to document: The field of study of your previous and current qualifications The country in which you took your previous and current qualifications The name of the Awarding Institution for your previous and current qualifications You may also be required to give the length of any course you are on and what year you are in ----------------------------------------- Stage 3 Linguistic Skills English skills (Reading / Writing / Verbal) : Portuguese skills (Reading / Writing / Verbal) : Catalan skills (Reading / Writing / Verbal) : Spanish skills (Reading / Writing / Verbal) : Italian skills (Reading / Writing / Verbal) : (If any of the above are your mother tongue you will not be asked about it) Don't forget to add a proof of your linguistic skills (copies of diplomas, certificates, etc) to your application documents. If you are not a native English speaker, you will be asked to make a Self Assessment of your English skills. Download the explanation sheet and read it carefully before filling the test. ----------------------------------------- Stage 4 Personal and Motivation Statement A Personal Statement of up to 2000 (Characters - this may vary as carriage returns tabs and other special characters may be counted as more than one character) This is to give the University Admissions a chance to get to know you, your aspirations, interests, experience and talents, professional, academic and personal. Describe what are your particular skills and abilities that make you an excellent candidate for this master. Describe any previous experience related to the subjects of the master, either academic or professional (if relevant, describe your current and/or previous employment, position and duration). In case you are author in scientific publications don't forget to mention it and send pdf copies and/or a separate list of publications. A Motivation Statement of up to 2000 (Characters - this may vary as carriage returns tabs and other special characters may be counted as more than one character) Explain why you are applying for this course. Include: * Your academic interests * Why you have chosen this subject * What interests you especially about the course * Link these ideas to your chosen career * Why are you interested in studying in Europe/ a third country * Why are you well prepared for living and studying in Europe/ a third country * Do you foresee any personal problems (family/ financial/cultural) NOTE: After submitting your Personal and Motivation Statements you will be asked to check wether the final text is ok. Please check that both texts are completely legible. Often when copying/pasting the text may appear cut. If this is the case, please upload the texts together with your certificates in Stage 7 ÒUpload documentsÓ. ----------------------------------------- Stage 5 References You will be required to give contact information of 2 referees. Your referees should be people who know you very well professionally, can attest your qualities, and preferably they should hold positions either in academia, research or industry in fields relevant for this master. You will be required to provide: Name Position (eg University lecturer at University of 'xxxxxxxx') Full address Telephone Number email ALL referees will be contacted with an email from emmcchir@ualg.pt. We strongly suggest that you speak to them before applying. Please provide only one email address per referee. This should be an institutional email address (not a personal hotmail, gmail, etc). Please write it carefully. Any mistake in this address will result in your application not receiving that reference! ----------------------------------------- Stage 6 Special Circumstances You will be required to indicate any Physical or learning disabilities that you may suffer from. These will be treated confidentially. ----------------------------------------- Stage 7 Upload documents You should upload .pdf copies of the following documents (max 1.5Mb each): - ID document (passport or other); - Diploma(s) and Transcripts of records. If you have more than one file, you should merge them into one document. - other certificates (optional): international English test certificates, etc. If you have more than one file, you should merge them into one. You may also upload your Motivation and Personal Statements her, if you found problems in Stage 4. If you have problems uploading a file, check if the size is above 1.5Mb. Large files will not be uploaded. If your file is larger than 1.5 Mb you may compress (zip) it into a smaller file, or decrease its resolution. If this does not help, you may send the files by email to emmcchir@ualg.pt, stating you application number. Please make sure you get a confirmation of reception of your files. ----------------------------------------- Stage 8 Summary Page This page simply summarizes all of the information you have entered, allowing you to make sure that it is correct before submitting. Note: take particular care checking that your Personal and Motivation Statements are complete, and that the email for contacting each referees is correct. These have been common sources of error in previous applications. ----------------------------------------- Stage 9 Thanks and print out page This page is to thank you for taking the time to complete the form. There will be another summary of the information which you must now print out and sign.
